Common Questions

  • How does a Crossover membership work?
    As an employee of Crossover Health, your annual XOXO membership fee is covered by Crossover for you and your dependents aged 18 and older on the Aetna health plan. This gives you access to all of the health services we provide at your “home” center as well as other nearsite Crossover centers you may choose to use (for instance, if one of our locations is closer to your home than your office). Furthermore, you will never receive a bill from us. You only pay the required co-pay amount, if any, at the time of your appointment as noted on the fee schedule for this employee health benefit. Some fees for our services will vary based on your deductible maximum out-of-pocket status, and all of this is kept current on your secure Crossover account so that you are charged accurately. In short, there is no complicated paperwork or insurance submittals with this benefit.
  • What happens if I miss my appointment or cancel with less than 24 hours notice?
    Canceling or rescheduling an appointment with less than 24 hours notice or failure to show up for an appointment will result in a missed appointment. While we will continue to be as flexible as possible, if you are more than 10 minutes late to your appointment, you may need to be rescheduled at your provider's discretion which will also result in a missed appointment. Missed appointments will result in a $30 fee.
  • What is the actual Missed Appointment Policy?
    Missed Appointment Policy

    First Occurrence: we understand unexpected conflicts and urgent matters are a part of life. As a courtesy, we will waive the $30 fee for your first missed appointment of the year.

    All Occurrences Thereafter: a $30 fee will apply for all missed appointments for the remainder of the year.

    Account Balances: in adherence with this policy, we require members pay their account balances to zero ($0) prior to future appointments at Crossover Health.

    Fee Charges: missed appointments will be charged to the credit card you have on file. You are agreeing that Crossover may charge your credit or debit card on file automatically in the event you miss an appointment. These charges will occur automatically if the conditions described above are met. Please note FSA/HSA cards may not be used to pay the $30 fee. If you do not have an eligible credit or debit card on file, Crossover will contact you directly for payment.

  • How are purchases for lenses and frames in the VSP-affiliated dispensary made?
    For Crossover Health members, the onsite VSP vision center can place your order for lenses and/or frames via your Guardian vision coverage. Lenses/frames may be purchased out-of-pocket if you don’t have VSP or you have exhausted your VSP benefit for the year.
