Diagnostic Home Testing Kit


This kit is designed to collect your saliva sample. Follow these steps to complete the test.

The test kit should only be used by the individual who ordered it; this will ensure accurate and ethical reporting of results.
Reminder: Do NOT eat, drink, smoke, or chew gum 30 minutes before giving your saliva sample.
If you ordered your test kit through the Crossover Passport app, or your employer ordered this test kit for you, log into the app and select “Take Test” prior to collecting your sample. You must complete the Public Health Survey within the app and register your collection device with the barcode scanner.
When you are ready to begin collecting your saliva sample, use the enclosed collection tube and complete steps 1 through 4.


Fill Out Your Information
Before starting the process be sure to write your date of birth in the blank space on the outside of the tube with the provided marker.
Fill the tube with saliva to the black wavy line.
Fill the tube until your saliva (not including bubbles) is at or just above the wavy line.
Replace the funnel with the fluid cap.
Remove the funnel and top the tube with the cap. Screw the cap on tightly—this releases solution in the cap that will stabilize the RNA in your saliva.
Firmly screw cap down to release solution and seal tube.
Check to be sure the solution released. You will know the cap is screwed on correctly when the blue solution from the cap is released into the tube. Cap must be firmly in place to assure the cap and tube are completely sealed.
Shake the tube for at least five seconds.
This will ensure your sample mixes thoroughly with the stabilizing solution.
When you have finished collecting your saliva sample, place the specimen tube into the biohazard bag. Place bag in the original box. Remove tape to expose adhesive and close flap. Bring pre-paid package to any UPS drop off location for shipment. Ship your package to the lab on the same day that you complete your test. Your results will appear in the Results section of the app.
